In signing this waiver, I release Elite Futbol Experience LLC and any appointed coaches, administrators, and/or other affiliated parties from any claims or causes of action that result from any harm or injuries suffered doing all soccer training sessions, drills, fitness, and/or camps.
In signing this waiver, I waive any right that I or the above-named participant may otherwise have to bring a legal action against Elite Futbol Experience LLC, and any appointed coaches, administrators, and/or other affiliated parties for personal injury. Because physical exercise can be subject to risk of serious injury, I knowingly assume all risks associated with participation in soccer training with Elite Futbol Experience LLC, including harm or injuries that result from the negligence of the participants or others involved with the program, including but not limited to Elite Futbol Experience LLC and any appointed coaches, administrators, and/or other affiliated parties.
In signing this waiver, in recognition that physical exercise can be strenuous, I verify that the above-named participant is in good physical health and has no restrictions on his or her ability to participate in any soccer training activity.
In signing this waiver, I give my permission for Elite Futbol Experience LLC to take and use photographs/videos of my child while participating in training programs and other events. These pictures/videos may be used for the sole purpose of promoting Elite Futbol Experience. These photos/videos may not be sold or distributed for other purposes.
In signing this waiver, I acknowledge that I have carefully read this "Waiver and Release" form and fully understand that it is a release of liability. I expressly agree to release and discharge Elite Futbol Experience LLC and any appointed coaches, administrators, and/or other affiliated parties from any claims or causes of action or any other right to bring a legal action for personal injury or property damage.
